Your First Acupuncture Session: What to Expect

If You Have Never Tried Acupuncture Before, You Are in Good Company

Nearly half of the patients who visit Piraluna are trying acupuncture for the first time. They come in curious but nervous, unsure what to expect, and often carrying assumptions picked up from movies or social media that have very little to do with reality.

This post will walk you through the entire experience, from before you arrive to after you leave. By the end, you will know exactly what happens during an acupuncture session and why there is nothing to worry about.

Before You Arrive

There are a few simple things you can do to get the most out of your first treatment.

Eat something light beforehand. Do not arrive on an empty stomach or right after a large meal. A light snack an hour or two before your appointment is ideal. This helps prevent lightheadedness and ensures your body has the energy it needs for treatment.

Wear comfortable, loose clothing. Your practitioner will need access to your arms, legs, and sometimes your back or abdomen. Loose shorts and a t shirt work well. If you are coming straight from the beach, that is perfectly fine too. We are in Koh Samui after all.

Avoid caffeine and alcohol. Both can affect your pulse, which is an important part of TCM diagnosis. Water and herbal tea are great choices before your appointment.

Show up a few minutes early. Your first session includes a longer consultation, so arriving a bit ahead of time helps us start without feeling rushed.

The Consultation

Your first visit begins with a conversation. Claire will ask you about the reason for your visit, your health history, your daily habits, your sleep, your digestion, your stress levels, and your emotional state. Some of these questions might seem unrelated to your main complaint. They are not.

In TCM, everything is connected. Digestion affects sleep. Emotions affect pain. Stress affects everything. By understanding the full picture, Claire can identify the underlying pattern driving your symptoms and create a treatment plan that addresses the root cause, not just the surface complaint.

She will also check your pulse. Not the way a Western doctor checks it (just counting beats per minute), but a detailed assessment of the quality, depth, speed, and rhythm of the pulse at three positions on each wrist. In TCM, the pulse provides a window into the state of your internal organs and energy.

She will also ask to look at your tongue. The color, coating, shape, and moisture of the tongue reveal information about your digestion, circulation, and overall constitutional balance. This might feel unusual the first time, but it is a quick and painless part of the assessment.

The whole consultation usually takes 15 to 20 minutes for a first visit. Follow up sessions are shorter since the baseline has already been established.

The Needles: What They Actually Feel Like

This is the part most people worry about. Let us be direct: acupuncture needles are not like the needles you know from injections or blood draws.

An acupuncture needle is about the thickness of a human hair. You could fit ten to fifteen acupuncture needles inside a standard medical syringe needle. They are solid, not hollow, and they are designed to slide smoothly through the skin rather than cut it.

When a needle is inserted, most people feel one of the following:

  • Nothing at all. Many points are placed without the patient feeling anything.
  • A brief, light pinch. Similar to a mosquito bite, lasting less than a second.
  • A dull ache or warmth. This is called "de qi" (pronounced "duh chee") and it means the point has been activated. It is a good sign.
  • A tingling or spreading sensation. Some patients describe energy moving along a line from the needle point. This often follows the meridian pathway.

What you will not feel is sharp, sustained pain. If anything feels uncomfortable, tell your practitioner immediately and she will adjust or remove the needle. You are always in control.

During the Session

Once all the needles are placed (typically 8 to 20, depending on your treatment plan), you will rest quietly for 20 to 40 minutes. The lights will be dimmed. You might hear soft music or nothing at all. This is your time to simply be still.

What happens during this resting period is remarkable. Your nervous system shifts from sympathetic (fight or flight) to parasympathetic (rest and restore). Your breathing slows. Your muscles release. Many people enter a deeply relaxed state that is somewhere between waking and sleeping. Some patients snore. This is completely normal and actually a sign that the treatment is working.

Claire stays nearby during the entire treatment. If you need anything or feel uncomfortable at any point, she is right there.

Depending on your condition, she may also incorporate other TCM techniques during your session:

  • Cupping uses suction cups on the back or shoulders to release muscle tension and improve circulation.
  • Moxibustion involves warming specific points with an herb called mugwort to strengthen energy and promote healing.
  • Gua sha is a gentle scraping technique that relieves tension and stimulates blood flow.

Claire will always explain what she is doing and why before adding any technique to your session.

After the Session

When the needles are removed (which you rarely feel), most people describe feeling calm, relaxed, and slightly floaty. Some feel energized. Others feel pleasantly tired. All of these responses are normal.

Here is what to keep in mind for the rest of the day:

  • Drink water. Staying hydrated helps your body process the changes from treatment.
  • Avoid intense exercise. A gentle walk is fine, but skip the gym or heavy workout for the rest of the day.
  • Do not drink alcohol. Give your body the evening to integrate the treatment.
  • Pay attention to how you feel. Some people notice immediate improvement. Others feel the effects over the next day or two. Occasionally, symptoms may briefly intensify before improving. This is a recognized part of the healing process and usually resolves within 24 hours.

Many first time patients tell us they slept better than they have in weeks on the night of their first treatment. Even if you came in for pain or stress rather than sleep, this is a common and welcome side effect.

How Many Sessions Will You Need?

This varies widely depending on your condition. Acute issues (a recent injury, a stiff neck from travel) may respond in one to three sessions. Chronic conditions that have been present for months or years typically require a longer course of treatment, usually six to twelve sessions.

Claire will give you an honest estimate during your first visit. She will also tell you if she thinks acupuncture is or is not the right approach for your specific situation. There is no pressure to commit to a long treatment plan. You can try one session and decide from there.

Common Questions

Does acupuncture hurt? Rarely. Most patients are surprised by how little they feel. The sensation is nothing like getting a shot or having blood drawn.

Is it safe? Yes. Acupuncture is extremely safe when performed by a trained practitioner using sterile, single use needles. At Piraluna, every needle is used once and disposed of immediately.

What if I am afraid of needles? Many of our patients were nervous about needles before their first session. The needles used in acupuncture are so thin and the insertion is so gentle that most needle phobic patients are surprised by how comfortable the experience is. Claire is experienced with anxious first timers and will go at your pace.

Can I get acupuncture if I am pregnant? Yes, but certain points must be avoided during pregnancy. Always inform your practitioner if you are or might be pregnant.
